Edit pulls, issues and comments; delete comments

A pull's or issue's title and body, and a comment's body, could not be
changed once written, and a comment could not be taken back: the web
pages only read, and nothing else wrote them. Their author can now
change them (`knotless edit`, the `edit_item` tool), which rewrites the
record in their account and stamps `updatedAt`, and delete a comment or
review (`knotless delete-comment`, the `delete_comment` tool). Pulls and
issues are closed rather than deleted, which would orphan what was said
about them.
